Lucy,
each air carrier has its FAA-approved manual and guidance regarding the transportation (and "storage") of animals. Some air carriers have taken these policies and interpreted/executed them in their own ways. There are plenty stories of how pets were "pampered" (in respect to their storage) on aircraft. Ask the Flying Dogs.
Nowadays it has become more compliant than before.
BTW, one of my cats once (quite a few moons ago) had a free ride to Walkers, and the pilot realized only at the island, that he had my airport cat on board. How to bring it back? Well, end of the story is, the cat was able to make it back to the airport. Today, no more.
For your dog and air travel. Appropriate crate size and in she goes (but still in the cabin) as there are no aircraft (of the smaller air charter carriers) to my knowledge that have a cargo hold.
For larger aircraft, there is a cargo hold on Gulfstream (Beech 1900), American Eagle (ATR-42 or-72) and Bahamas Air (Dash-8).
